Check the status of your case
Check the issuance/registration status of the EU medical entitlement document
Are you waiting for an EU document entitling you to medical benefits under the provisions on coordination of social security systems, issued by a provincial branch of the National Health Fund or an institution of another EU/EFTA Member State? If the document was sent through the Electronic Exchange System of Information (EESSI), then you can keep track of the status of your case.
Simply enter the case ID number in the case search field. You will receive the identification number on your e-mail given in the application for an authorization document or on the statement of its registration.
This includes documents related to confirmation of the right to medical treatment during a temporary stay (S044) or transfer of residence (S1/S072), as well as documents related to confirmation of past insurance periods (S040) or the right to benefits for an accident at work or occupational disease (DA1/DA002).
Request a case identification number
If you do not have a unique identification number, you can apply for one. Download and fill out an application for its issuance[1]. You can submit your application to the NFZ branch or delegation responsible for you:
- in person
- via the Electronic Platform for Public Administration Services (ePUAP)
- via e-mail, if you can sign the application with your electronic signature[2].
Important! Your data is safe! The system provides only anonymous information, i.e. the name of the document, the stage and the status of the case.

[1] Does not apply to EHIC applications - in this case, the case ID is sent automatically to the email address you specify at the time of application.
[2] Sign with a personal signature, qualified electronic signature or non-qualified signature (as long as the signature provider is listed in the register of non-qualified trust services maintained by the National Certification Center) and transmit as an electronic file.
