Obtain the EHIC using the Internet Patient Account (IKP)
Step 1: Go to your IKP account
1. Visit the website: https://pacjent.gov.pl.
2. Log in to your account.
3. Go to ‘My Account’.
4. Below your health insurance information you will find information about the EHIC card and the ‘Apply’ button.
Step 2: Complete your application online
- If you need the EHIC to be labelled with a Braille sticker, please type: Card for blind/visually impaired person.
- Please provide the address where you want us to post the EHIC.
Step 3: Submit your application online
- Are you 18 years old, are you a pupil, student or doctoral student and are you insured as a family member? Please enclose with your application a copy of your current pupil/student ID card or other proof of continuing your education.
- We will process your EHIC application within 5 working days, and within 10 working days from June to September.
PLEASE NOTE! When applying using IKP, the EHIC is always posted to the address provided.
Obtain the EHIC via mojeIKP mobile app
Step 1
- Log in to the mojeIKP app.
- Go to the "Health insurance abroad" tab.
- Select "Application for EHIC card".
If you already have an EHIC at the time of application, the system will display information about the existence of a valid EHIC. The expected date of dispatch of the new EHIC and the expiry date of the card will also be indicated. If you do not agree with its expected expiry date, please attach a document to your application confirming that you are entitled to a longer expiry date, or contact the competent NFZ regional branch. The competent regional branch of the NFZ is the branch to which you are subject by virtue of your place of residence in Poland.
Step 2: Fill in the online application
- Complete the data and add the necessary attachments.
Are you 18 years old, a pupil, student or doctoral student and have insurance as a family member? Attach a copy of your current pupil/student ID card or other proof of continuing education to your application.
- Fill in the details of your stay.
Enter the address to which the EHIC is to be sent.
NOTE! If you apply through mojeIKP, the EHIC is always sent to the address you specify.
- Read the statements and give all consents.
Step 3: Submit your application online by selecting the 'submit application' button
We will send you information on the status of your application to the email address you provide in your application:
- the status completed means that the EHIC has been issued; you will receive a link to track the delivery of your EHIC card
- Rejected status means that an EHIC cannot be issued.
