Obtain your e-Certificate via the mojeIKP mobile app
Remember!
The certificate is issued in exceptional circumstances - when treatment is required.
Step 1:
- Log in to the mojeIKP app.
- Go to the "Health insurance abroad" tab.
- Select "Applications for Certificate".
Step 2: Fill in the online application.
- Complete the data and add the necessary attachments.
Are you 18 years old, a pupil, student or doctoral student and have insurance as a family member? Attach a copy of your current pupil/student ID card or other proof of continuing education to your application.
- Fill in the details of your stay.
Are you travelling for work? Attach, as appropriate, an A1 certificate (issued by ZUS or KRUS) or a U2 document (issued by the Unemployment Office).
- Read the statements and give all consents.
Before submitting your application, the system will display the expected timeframe for processing your application.
Step 3: Submit your application online by selecting the 'submit application' button.
You will receive information about the issued e-certificate or its refusal at the e-mail address indicated in the application.
NOTE! If you apply via the mobile app, the finished e-certificate will be available for download from your mojeIKP account.
Where to find the final e-Certificate:
- Go to the "Health insurance abroad" tab.
- Go to the list of " Applications for Certificates" and then find the application with the status completed.
- Click " view certificate".
- The issued e-Certificate is available in PDF format and bears a qualified seal.
